13-02 Version of 25.2 Application Methods

BadgerCare Plus applicants have the choice of one of the following applicationA request for BadgerCare Plus coverage. The request must be on the Department's application or registration form and must contain name, address, and a valid signature. The applicant must submit a signed and completed application form to complete the application process. methods:

  1. ACCESS https://access.wisconsin.gov/.

  2. Mail-In using the BadgerCare Plus Application Packet (F-10182).

  3. Telephone Interview.

  4. Face-to-Face Interview.

  5. Use of the paper or online application available through the Marketplace

  6. Telephone application with the Marketplace
