If a service provider does not agree with the decision regarding his or her application, the service provider may request a fair hearing. Requests must be made in writing to:

Hearing requests should include the service provider’s name, mailing address, signature, decedent's case number, a brief description of the issue, and which state agency took the action.
Note: Family members do not have appeal rights.
