Name of the individual with self-employment income and type of business reported in CARES. Type of self-employment business includes, but is not limited to:
Business Owner/Operator
Babysitting/Child Care
Carpentry/Construction
Rental Property
Displays the monthly self employment income amount from CARES. Select from the drop down box provided:
Yes = The gross income amount from CARES is correct.
No = The gross income amount from CARES is incorrect. Enter the correct amount into the "Correct Amount” field.
Displays the monthly gross self employment expense from CARES. Select from the drop down box provided:
Yes =The gross expense from CARES is correct.
No =The gross expense from CARES is incorrect. Enter the correct expense in the "Correct Amount” field.
Displays the monthly disallowed self-employment expense amount from CARES. Select from the drop down box provided:
Yes =The disallowed expense from CARES is correct.
No = The disallowed expense fron CARES is incorrect. Enter the correct disallowed expense amount in the"Correct Amount” field.
Displays the self employment income budgeted for the case. Select from the drop down box provided:
Yes =The total monthly average amount from CARES is correct.
No = The total monthly average amount from CARES is incorrect. Enter the correct monthly average amount into the "Correct Amount” field.
If any CARES self employment information is incorrect, select a discrepancy reason from the drop down box provided.
Determine if all self employment income was included and select Yes or No from the drop down box provided.
Yes = All self-employment income for this case was included.
No = Self-employment income was not reported and/or recorded in CARES. Select the appropriate discrepancy reason from the drop down box provided in field (9)
If all self employment income was not included in field (8), reviewer must select a discrepancy reason from the drop down box provided and enter the dollar amount of the error in the Comments section of the Healthcare Error Identification page and/or the FoodShare Discrepancies and Findings page.
