Is the liquid asset information from CARES correct? Reviewer must select Yes or No from the drop down box for each asset listed.
Yes = The amount is correct.
No = The amount is incorrect. If answer is no, enter the correct amount in field.
The correct amount entered by the reviewer when the answer to field (4) is No.
If any asset included for the review month is incorrect, the reviewer must select a reason from the drop down box that caused the asset to be incorrect. A discrepancy reason must be selected for each incorrect asset.
6a. The total asset amount reported in CARES.
6b. The updated total asset amount if the reviewer corrected any individual asset amount.
Reviewer must determine if all assets have been included correctly.
Yes: Reviewer found that all assets were included.
No: Reviewer determined that an asset was not included that should have been.
N/A: This question doesn’t apply to the case being reviewed.
When the answer to field (9) is No, the reviewer must select a discrepancy reason from the drop down box that caused it to be incorrect. Enter dollar amount of error in the Comments section of the Healthcare Error Identification page and/or the FoodShare Discrepancies and Findings page.
